How new vaccine strategies are moving from prevention to personalized treatment

This edition explains a phase III milestone for a personalized melanoma vaccine, evidence supporting broader HPV vaccination of young men, and the opening of NHS winter flu and COVID-19 vaccination bookings.[3][4][6][10]

How does a personalized mRNA vaccine target melanoma recurrence?

The trial enrolled around 1,100 people whose advanced melanoma had been completely removed and compared intismeran plus pembrolizumab with pembrolizumab alone.[3] The combination met endpoints for recurrence-free survival and distant metastasis-free survival, but detailed phase III efficacy and safety results have not yet been released.[4]

Why it matters

This is the first personalized mRNA cancer treatment to report success in a late-stage trial, providing evidence that vaccines tailored to tumour mutations can work at scale.[3] Its clinical value remains uncertain until researchers disclose the magnitude and durability of the benefit, adverse events and longer-term survival outcomes.[3][4]

Key insights

  • Intismeran is intended to prevent melanoma from returning after treatment, not to prevent someone from developing cancer initially.[3]
  • A patient's tumour is sequenced to identify mutations and corresponding neoantigens; the customized mRNA then instructs the body to produce selected neoantigens and trains the immune system to recognize the tumour.[3]
  • Earlier phase IIb results associated the combination with a 49% lower risk of recurrence or death and a 59% lower risk of distant metastasis or death, but those figures are not the still-unreleased phase III effect sizes.[4]
  • Producing an individualized vaccine can take several months, creating a practical barrier for patients with rapidly progressing advanced cancer.[3]
